This is a neck dimple and is commonly called a thumbprint and some call it the thumbprint of God. These can be found anywhere on the neck and is a common birth anomaly.
Natural dimples in the neck of this horse are commonly found in newborn horses. They are incidental findings and I often used them in identification for the Coggins test.
